Mortuary Passport

Issuance of the mortuary passport is free of charge

Examination period – 3 working days

The body of the citizen of the Republic of Moldova can be transported from the USA to the Republic of Moldova only with the prior obtaining of the mortuary passport issued by the Embassy of the Republic of Moldova to the USA. The mortuary passport is issued in accordance with the provisions of the Agreement on the transfer of the bodies of deceased persons, concluded in Strasbourg on October 26, 1973, ratified by Law no. 1451 of November 8, 2002. The request for the mortuary passport can be made by a family member or the representative of the funeral home in the USA that is directly in charge of transporting the body to the Republic of Moldova. The mortuary passport is issued exclusively for the transport of bodies.

In the case of transporting the funeral urn, approvals from the Embassy are NOT required and a mortuary passport is NOT issued. It is necessary to have the US death certificate in the original legalized with an apostille at the Secretary of State of the state issuing the death certificate, as well as the cremation/incineration certificate.

The following documents are required to issue the mortuary passport:

  • Letter / application requesting to obtain the mortuary passport from the funeral company / relative of the deceased person (a copy of the identity document / passport of the applicant will be attached);
  • The Moldovan passport of the deceased person, in original; If the deceased person also held other citizenships, a copy of the respective passport will be presented;
  • The death certificate issued by the US authorities in the original, legalized with the Apostille at the Secretary of State of the state issuing the death certificate. The certificate must contain the cause of death;
  • Confirmation that the deceased did not suffer from an infectious or contagious disease and does not pose a danger to public health – it is issued by the funeral company that prepares the documents for transport;
  • The Embalming certificate – it is issued by the funeral company that prepares the documents for transport;
  • Proof that the casket was properly sealed and does not pose a danger to the environment – it is issued by the funeral company that prepares the documents for transport;
  • Prior authorization for burial (releasing the body from the morgue) – if the death occurred as a result of a violent action or for unexplained reasons;
  • The itinerary of the coffin – can be mentioned in the letter/request, specifying the transport route, the city and state of departure from the USA, the city and country of transit and the city of arrival in the Republic of Moldova;

When the package of documents is sent by mail, a prepaid USPS return envelope with tracking number and delivery address is added for the return of the original documents and mortuary passport.

No consular fees are charged for the issuance of the mortuary passport.

For applications submitted in person, the settlement can be done on the same day. Applications will be submitted during business hours and on days when the Embassy provides consular support to citizens with appointments.
