Duplicate civil status certificates

Service duration: 15 minutes
Appointment to this service is mandatory for files submitted in person: one applicant = one appointment. The corresponding file can also be sent by post.

 
Duplicate certificate / extract from the certificate of birth, marriage, divorce, death or change of name and/or first name.

The service for issuing the certificate/duplicate certificate/extract from the civil status document is part of the category of services for issuing civil status documents whose purpose is to confirm the registration of the civil status document and which influence the appearance, modification or termination of the rights and obligations of individuals and characterizes the legal status of the respective civil status fact.

Duplicates of civil status documents are issued for permanent use and are not subject to any retention (withdrawal), except in the cases expressly established by the legislation. Civil status certificates are withdrawn in the conditions of their issuance contrary to the data of the Civil Status Acts (being assessed as non-compliant) or in the conditions of identification of their falsification after issuance.

Duplicates of civil status certificates are issued in the following cases:

  • when the certificate is damaged;
  • upon loss of the certificate;
  • when requesting the new model certificate in connection with its transcription in Latin script.   

Duplicates of civil status certificates are issued upon request:

  • to the holder of the civil status document or in exceptional cases (argued in writing) to his first degree relatives, on the direct line; Civil status documents confirming the degree of kinship will be presented.
  • to family members, relatives of the deceased or other entitled persons - in the event that the holder of the civil status document has died; Civil status documents confirming the degree of kinship will be presented.
  • the legal representative of the minor, the person limited in the exercise capacity or declared incapable by the court decision, the parents, guardians or curators, the representative of the guardianship authority;
  • to the representative authorized by or power of attorney.

To request duplicates of civil status certificates, the applicant will present:

  • Appropriate application completed in full; (download the application here – birth certificate, marriage certificate, divorce certificate, death certificate, data change certificate.
  • Valid identity card or valid Moldovan passport in original;
  • Copy of the Moldovan certificate of birth, marriage, divorce, death, as applicable;
  • In the case of the foreign citizen – the notarized copy of the valid US passport, ID, driver's license;
  • Civil status documents confirming the degree of kinship will be presented, if applicable.
  • For requests sent by mail, the file will be completed with a prepaid USPS envelope with tracking number, and the return address;
  • Confirmation of payment of consular fees.

The application and the corresponding file are to be sent to the Public Services Agency, which examines the application and, if necessary, completes the document and then sends the certificate to the Embassy.

After receiving the documents from the Republic of Moldova, they will be sent by mail to the applicant's residence if such an option was chosen at the time of submitting the documents and a USPS prepaid envelope with tracking number and return address was presented. Otherwise, the documents will be obtained personally from the Embassy;

The documents issued by the Public Services Agency are not legalized with an apostille. In order to be recognized on the territory of the USA, Moldovan civil status certificates must contain the Apostille applied to the document, with the translation of the document in English and the Apostille on the translation (if the translation was made in the Republic of Moldova). In order to be recognized on the territory of the Republic of Moldova, the documents issued by the US authorities must contain the Apostille applied to the document. The Apostille service for documents issued by the Moldovan authorities is ONLY available in the Republic of Moldova - the Ministry of Justice.
